Sef in the link I provided you you need to select "Account/Billing", then "PlayOnline ID/Password Recovery" or "Compromised Account" (up to you) and then write something like "I cannot access my PlayOnline account".
In the next page you select "Next" and then you select "Proceed to Chat Support" which is avaible Monday through Friday 9:00 - 18:00 (Pacific Time) (Excluding holidays).
Then you need to fill the blanks and you'll be in the chat room waiting for a SE agent to pick you up D:

You need ID, POL Codes (FFXI and expansions), name of the original owner, address of the original owner, birthdate of the original owner, original cc code (first 4 digits and last 4). You don't need to provide to them all of these infos, it's just better to have most of them D:
Also they'll ask for an ID and a pic of the codes.
You don't need the original password for the account <.<
