They'll ask you these infos:
- Full name you used to register to POL
- Birthday
- Address + Postal code
- (If using VISA) First 4 digits and last 4 of the cc you used to pay for FFXI
- POL registration code and FFXI codes (with all the expansions you registered)

Also they might ask you to send via mail your ID (driver license or passport) and a pic of your POL code.
